LVISD Back To School Forms

2020/2021 School Year

LVISD has transitioned all back to school forms to a digital platform for the 2020/2021 School Year. Forms are listed by district, and by campus.

District forms are forms that you can complete for each of your students simply by "adding a child" on the first screen for the form. You will then be asked questions for each child that you have added. (Video Example)

Campus forms are forms that are specific to that campus. Please complete the campus forms that are relevant for your student(s).

District Forms

• Health Services Form: ALL LVISD students are required to have a student health information form completed. If you have questions please contact your student's campus nurse. 
  
  
• Acceptable/Responsible Use of the Districts Technology Resources: ALL LVISD students are required to have an AUP form on file for the 2020/2021 school year. If you have questions please email technology@lvisd.org or call 830-779-6657.
  
  
• LVISD Web Publishing form: All LVISD students are required to have a Web Publishing form on file for the 2020/2021 school year. If you have questions please email technology@lvisd.org or call 830-779-6657. 
  
  
• Acknowledgment of Electronic distribution of Student Code of Conduct and Student Handbook: All LVISD students are required to have a code of conduct and handbook form on file for the 2020/2021 school year. Please contact the campus with questions regarding this form.
  

To be completed based on the need for service:   

If you have already arranged for transportation services with our Transportation Team there is no need to resubmit this form.                                                           

• Transportation Form: If you would like to request transportation services for your student(s) please complete this form. If you have questions regarding the transportation form you can contact: bus@lvisd.org via email or call 830-779-6610. 

• LVISD Free/Reduced Lunch form: information provided on this form is confidential.

Campus Forms

Primary Campus & Intermediate Campus:

YMCA Student Drop-In Service Form: this form allows your student to be taken to the LVISD YMCA after school care program when a transportation emergency arises.  The drop in form is not registration in the program.  To register for YMCA After School care, please go to YMCA After School Care LVISD. 

Junior High School Campus & High School Campus: 

Acknowledgment of Electronic Distribution of LVISD Extracurricular Student Code of Conduct form. This form is required for all who participate in extracurricular activities.

LVISD Drug Testing Consent Form. This form is required for all who participate in extracurricular activities and want to drive/park on campus.
